Early Orthodontic Evaluation: When Should Your Child See an Orthodontist?

Have you noticed your child covering their smile in photos? Maybe you’ve spotted crowding, spacing issues, or a bite that doesn’t look quite right. As pediatric orthodontic specialists here at McKinney Dentist, we hear one question more than any other: “Is my child too young for an orthodontic evaluation?”

Here’s the truth: Your child is never too young for a screening. The American Association of Orthodontists recommends children have their first orthodontic evaluation by age 7—not because most kids need treatment that early, but because this is the ideal time to identify issues that might benefit from early intervention.

We know age 7 sounds young. Many parents tell us they expected to wait until their child lost all their baby teeth. But in our practice, we’ve found that some problems are easier to correct when we catch them during active growth. Other times, we simply monitor development and reassure families that everything is progressing normally.

Why We Screen at Age 7 (Even If Treatment Isn’t Needed Yet)

Think of an early orthodontic screening like a wellness check for your child’s bite and jaw development. At age 7, children typically have a mix of baby and permanent teeth—what we call “mixed dentition.” This gives us a unique window to evaluate how the jaws are growing and whether permanent teeth will have enough room to erupt properly.

During this screening, we look for:

  • Crossbites that could affect jaw growth if left untreated
  • Severe crowding that might require extraction of permanent teeth later
  • Protruding front teeth that are vulnerable to injury (sports, playground accidents)
  • Bite patterns that could lead to excessive wear on teeth
  • Jaw discrepancies that are easier to address during growth spurts

Here’s what we don’t do: push unnecessary treatment. If your child’s development looks great, we’ll tell you. If we identify a concern, we’ll explain exactly what we’re seeing, why it matters, and whether we should intervene now or monitor and wait.

Warning Signs That Warrant an Earlier Evaluation

While age 7 is our general recommendation, some children benefit from being seen earlier. Call us at McKinney Dentist if you notice:

Functional Issues:

  • Difficulty chewing food properly
  • Speech problems that may relate to tooth or jaw position
  • Mouth breathing instead of nose breathing
  • Jaw pain, clicking, or shifting when opening or closing

Visible Alignment Problems:

  • Severe crowding where permanent teeth can’t erupt properly
  • Crossbite (upper teeth bite inside lower teeth)
  • Underbite (lower jaw extends past upper jaw)
  • Significant overbite or overjet (front teeth protrude)

Habits That Affect Development:

  • Thumb-sucking or pacifier use continuing past age 5
  • Tongue thrusting
  • Early or late loss of baby teeth

We understand dental anxiety is real—for kids and parents. Our team at McKinney Dentist has extensive experience working with children, and Dr. Anthony Callison specializes in pediatric orthodontics. We take time to explain everything in language your child understands, and we never proceed with treatment unless you’re completely comfortable with the plan.

Understanding Two-Phase Orthodontic Treatment

One area where we see a lot of confusion is the concept of “Phase 1” and “Phase 2” treatment. Let’s break this down clearly:

Phase 1 Treatment: Creating the Foundation (Typically Ages 7-10)

Not every child needs Phase 1 treatment. We only recommend it when we identify specific problems that will be significantly easier to address during active growth, or when waiting would lead to more complex treatment later.

Phase 1 focuses on:

  • Expanding narrow upper jaws to create space for permanent teeth
  • Correcting crossbites before they affect jaw development
  • Reducing severe overbites or overjets that put front teeth at risk
  • Guiding jaw growth for children with underbites
  • Breaking harmful oral habits like thumb-sucking

Important reality check: Phase 1 doesn’t eliminate the need for Phase 2 in most cases. What it does do is address skeletal and developmental issues that we can’t correct as easily once growth is complete. Treatment length varies—some cases take 6-9 months, others 18-24 months, depending on complexity.

After Phase 1, there’s typically a “rest period” where we monitor development as remaining permanent teeth erupt.

Phase 2 Treatment: Final Alignment (Typically Ages 11-14)

Once all (or most) permanent teeth have erupted, Phase 2 treatment fine-tunes alignment and perfects the bite. This is when we create that finished, confident smile.

Phase 2 may involve traditional braces or Invisalign Teen, depending on your child’s specific needs and maturity level. Because Phase 1 addressed the foundation, Phase 2 is often shorter and less complex than if we’d waited to treat everything at once—though we want to be honest that this isn’t guaranteed in every case.

Invisalign for Kids: Is Your Child a Good Candidate?

We offer Invisalign for kids and teens at McKinney Dentist, and when it’s the right fit, patients love it. The clear aligners are nearly invisible, removable for eating and sports, and make it easier to maintain good oral hygiene compared to traditional braces.

But here’s the critical factor: compliance.

Invisalign only works if your child wears the aligners 20-22 hours per day. We’ve seen treatment fail when kids remove aligners frequently or “forget” to wear them. Before we recommend Invisalign, we have an honest conversation with you and your child about responsibility and commitment.

Your child might be a great Invisalign candidate if they:

  • Are self-motivated and responsible
  • Are concerned about the appearance of metal braces
  • Participate in contact sports or play wind instruments
  • Have good oral hygiene habits
  • Understand that aligners must stay in except when eating or brushing

We use advanced 3D imaging technology to show you and your child exactly how treatment will progress. This visualization helps kids stay motivated—they can literally see their future smile.

Our Approach: Making Orthodontics Comfortable for Kids

Let’s be real: most kids aren’t thrilled about dental appointments. As parents ourselves, we get it. That’s why we’ve built our practice around creating genuinely positive experiences for children.

Here’s what you can expect at McKinney Dentist:

We never rush. Dr. Callison and our team take time to explain procedures in kid-friendly language and answer every question—no matter how small. We’ve found that when children understand what’s happening and why, anxiety melts away.

Our McKinney office is designed with families in mind. We want your child to feel comfortable from the moment they walk through the door.

We celebrate progress. Orthodontic treatment is a journey, and every milestone matters. Whether it’s getting their first set of aligners or seeing significant tooth movement, we make sure your child feels proud of their progress.

We’re honest about what treatment involves—no surprises, no scary moments. We explain any discomfort they might experience (usually mild soreness for a day or two after adjustments) and how to manage it.

Schedule Your Child’s Complimentary Orthodontic Evaluation

Early orthodontic screening isn’t about rushing into treatment—it’s about giving your child the best possible foundation for lifelong oral health. Sometimes that means early intervention. Sometimes that means monitoring and reassurance. Either way, you’ll have the information you need to make confident decisions about your child’s care.

FAQ Section

At what age should my child have their first orthodontic screening?

We recommend children have an initial orthodontic evaluation by age 7, when they have a mix of baby and permanent teeth. This doesn’t mean treatment will start at age 7—in fact, most children we screen at this age simply need monitoring. But it does give us a baseline to track development and identify any concerns early, when they’re often easier to address.

What happens during an early orthodontic evaluation?

Dr. Callison will examine your child’s teeth, bite, and jaw relationship. We’ll look for crowding, spacing issues, crossbites, and jaw discrepancies. We’ll also discuss any oral habits like thumb-sucking. If we identify concerns, we’ll explain what we’re seeing and whether we recommend treatment now or monitoring for future treatment. If everything looks good, we’ll tell you that too—and recommend when to return for a follow-up check.

Does early intervention mean my child won't need braces later?

Not necessarily, and we want to be upfront about this. Phase 1 treatment addresses specific developmental and skeletal issues, but most children who receive Phase 1 treatment will still need Phase 2 treatment (braces or Invisalign) once all permanent teeth have erupted. However, Phase 1 often makes Phase 2 shorter and less complex. We’ll discuss realistic expectations for your child’s specific case.

How do I know if my child is responsible enough for Invisalign?

Great question. Invisalign requires wearing aligners 20-22 hours per day—they only come out for eating, brushing, and flossing. If your child is self-motivated, responsible about daily tasks, and committed to their treatment, Invisalign can work beautifully. If compliance is a concern, traditional braces might be more effective since they work 24/7 regardless of your child’s cooperation. We’ll help you make this decision during the consultation.

Will early orthodontic treatment hurt?

Most orthodontic treatment causes mild discomfort rather than pain—like the soreness you might feel after a workout. Children typically experience some tenderness for a day or two after getting new appliances or adjustments. Over-the-counter pain relievers and soft foods usually manage any discomfort easily. We’ll give you detailed instructions on what to expect and how to keep your child comfortable.

How much does early orthodontic treatment cost?

Treatment costs vary significantly depending on your child’s specific needs. Some Phase 1 treatments are relatively simple and shorter, while others are more complex. During your complimentary consultation, we’ll discuss treatment options and provide transparent pricing. We also offer flexible payment plans to make orthodontic care accessible for your family. Many insurance plans provide orthodontic benefits that can help offset costs.
